The occasion marks the second time McIlroy has claimed the Irish Open in his storied career, with the last coming in 2016.
"I feel just so lucky that I get to do this, I get to do this in front of these people," McIlroy said at The K Club following the round while being serenaded by the patrons. "The support has been absolutely amazing all week."
He added: "I love coming home, I love playing in this atmosphere. Moments like this, these are the things you're going to remember well after your career is over. This is a really special day."
McIlroy tied Lagergren on the 72nd hole with an electrifying 28-foot eagle putt to send the crowd into a frenzy.

He began the round four shots off the lead but fired a 6-under 66 to draw even with Lagergren, who sought his second DP World Tour victory and first since 2018.
McIlroy now ranks 12th all time in victories on the European circuit.
It's the fourth win overall in 2025 for McIlroy, who triumphed at the Pebble Beach Pro-Am, The Players Championship, and the Masters, with the latter two also ending in playoffs.
