Suns guard Devin Booker fouled Conley with 1:41 remaining in the final frame, causing the latter to fall into his 54-year-old head coach. Finch had to be helped to the locker room and did not return to the bench.
The Timberwolves completed a series sweep of Phoenix, winning Game 4 122-116 behind 40 points from Anthony Edwards.
Finch was a finalist for the Coach of the Year award, which was awarded to the Oklahoma City Thunder's Mark Daigneault earlier Sunday.
