Joshua Van will also be tasked with defending his flyweight championship belt against Tatsuro Taira in the co-main event.
The card's other notable fights include Curtis Blaydes vs. Josh Hokit with Patricio Pitbull vs. Aaron Pico and Mateusz Gamrot vs. Esteban Ribovics on the prelims.
Prochazka, 33, previously held the light heavyweight belt after beating Glover Teixera at UFC 275. The Czech fighter later had to vacate the belt and failed to win it back from Pereira on two separate occasions.
Since his most recent loss to Pereira, Prochazka has rattled off two straight wins, knocking out Jamahal Hill and Khalil Rountree Jr.
Ulberg, 35, is 14-1-0 for his career and is riding a nine-fight win streak that includes five KOs and a submission.
Pereira vacating his 205-pound division belt will only ramp up speculation that the 38-year-old Brazilian is looking to move up to heavyweight and become the first three-division champ in UFC history. He is also a former middleweight titleholder.
