Juan Cuadrado was forced to leave in the first half of Chelsea's season finale against Sunderland after suffering an apparent leg injury.
The team have yet to confirm the nature of his injury, but the Colombian appeared to be holding his left leg when the team's medical staff arrived to treat him.
It was only his third start of the season since joining Chelsea from Fiorentina during the January transfer window.
He was forced to leave the game in the dying minutes of the first half and was replaced by Loic Remy.
Although it was a disappointing end to his first season in England, Cuadrado became the first Colombian player to win the Premier League as Chelsea hoisted the trophy following their 3-1 win.
