Anders Dreyer's goal in the 72nd minute gave top seed San Diego FC a 1-0 home victory over Minnesota United on Monday to advance in the Major League Soccer playoffs.
The 27-year-old Danish international winger blasted in left-footed into the top corner for the winner.
San Diego now get a host berth in Saturday's Western Conference final against the second-seeded Vancouver Whitecaps.
The winner will face the Eastern Conference champion -- either Lionel Messi's Inter Miami or New York City FC -- in the MLS Cup final on December 6.
First-year expansion club San Diego topped the Western Conference in the regular season, with Dreyer scoring 22 goals and assisting on 16 others.
