Eden Hazard's time at Chelsea will apparently come to an end this summer, as he will inform the team of his desire to join Real Madrid, according to Matt Law of The Telegraph.

His reported intention to move on from Chelsea was revealed shortly after news broke that Hazard told French radio station RMC in an interview that his future is already decided.

"I know what I am going to do. I have made a decision," Hazard said in a documentary that will air in France on Wednesday.

Law reports that talks over a new Chelsea contract have stalled and that it's understood Hazard's willingness to leave comes after Real Madrid told him they intend to launch a bid to sign him.

Hazard has yet to reveal his plans to the club, but Law reports the 28-year-old will officially inform Chelsea officials that he wants to leave this summer.

Last week, Chelsea manager Maurizio Sarri conceded that he will not prevent Hazard from leaving the club.

"Eden is 28. If he wants to go, I think he has to go," Sarri said.

Although Hazard will only have one year left on his contract this summer, it's believed Chelsea will not waver from their £100-million valuation.

Hazard has starred for Chelsea since his arrival from French side Lille in 2012. Along with winning a pair of Premier League titles and the Europa League, the Belgian was also named the PFA Player of the Year in 2015.

The versatile playmaker is in the midst of another stellar season, as he has found the back of the net 12 times and provided 10 assists in 24 Premier League appearances.
