Patrick Bamford's nomadic existence is about to end.

The Chelsea loanee, who spent brief spells at various clubs over the past few years, will undergo a medical at Middlesbrough on Monday before finalising a permanent move to the Riverside, according to the Guardian's Stuart James.

The deal is believed to be worth as much as £10 million, including add-ons, but the initial fee is closer to £5 million.

Having joined Middlesbrough on a season-long loan for the 2014-15 campaign, Bamford is familiar with Aitor Karanka's side. He scored 17 league goals that term as Boro reached the Championship playoff final, and won the Championship Player of the Year award for his efforts.

Temporary stays at Crystal Palace, Norwich City, and most recently Burnley did not work out so well: He failed to score a single Premier League goal for any of those sides.

Chelsea signed Bamford in 2012 from Nottingham Forest as a teenage striker for a reported £1.5-million fee. Before that, he had only played 12 minutes of first-team football with Forest in the Championship.

The Blues kept Bamford in their orbit for the next four years, shipping him to lesser clubs in the hope he would find his feet.

Chelsea also gave the Lincolnshire native a new three-year contract in July 2015, but he never ended up playing a single minute of competitive football with the senior team.

The west London outfit recalled the 23-year-old from Burnley on the weekend, sparking reports of a permanent sale. He made just six appearances as a substitute this season.
